Banting Legacy Scholarship 2018 – 19
Guidelines for Applicants
The Sir Frederick Banting Legacy Foundation is pleased to provide a scholarship in support of youth living with diabetes who wish to pursue post-secondary education.
$3,000 payable in two instalments in the first academic year subject to confirmation of enrolment and continuing program participation.
Open to applicants who are:
a) permanent residents in Canada;
b) in the final year of high school;
c) attending a high school in Ontario or a current student of an Ontario school board;
d) entering into a post-secondary educational facility in the Fall of the year in which the Scholarship is offered;
e) have been living with either Type 1 or Type 2 diabetes for at least 2 years and are registered with a diabetes program or healthcare provider;
f) have fulfilled all the requirements for this scholarship listed below.
a) Completed Application Form;
b) Demonstrated competence in self-management of their condition as evidenced by two supporting letters: one from a member of their health care team and one from a non-relative adult;
c) An essay (1,000 words maximum and typed) in response to ONE of the topics listed on the application form.
a) Applicants will be judged on the basis of the information provided
in the application, personal essay, and supporting letters.
b) All applications received by email or regular mail post marked on
or before the application deadline will be reviewed by the SFBLF
Education Committee. The Committee will select the scholarship
c) SFBLF reserves the right not to award a scholarship in any given year.
